Documents required
To apply for an Ex-Servicemen Contributory Health Scheme (ECHS) card, you’ll need to provide the following documents:
Identification: Aadhaar card and PAN card
Proof of service: Discharge book
Photographs: Recent passport-sized photos of you and your dependents
Blood group: Your blood group and that of your dependents
Bank account: Bank account statement and IFSC code
Pension details: Pension payment order (PPO) number
Address: Permanent and temporary address
Dependent information: If applicable, PAN cards, birth certificates, and other details for dependents
BENEFIT
ECHS cardholders can receive allopathic and AYUSH treatment at empanelled hospitals and ECHS Polyclinics across India. They can also access outpatient care at 427 Polyclinics in India and six Polyclinics in Nepal.

The ECHS card stores medical history, past hospitalizations, and other details about the cardholder and their dependents. It also has built-in security features.
How to use
Cardholders must produce their ECHS card to receive treatment at empanelled healthcare providers and polyclinics. These centers can refuse to provide services if the card is not produced.
Application
Beneficiaries can apply for the ECHS card and receive a temporary slip in the meantime. They can register for the scheme using the ECHS beneficiary app or the website.
