Documents Required

A recent color passport-sized photograph 

  • Scanned copy of address proof, such as an Aadhaar card, driving license, or state domicile 
     
  • Scanned copy of identity proof, such as an Aadhaar card, PAN card, or driving license 
     
  • Disability certificate, if you already have one 
     
  • Signature, if you want to include it

Important Information

  • Online and offline applications:Registration applications can be submitted online or offline.Offline applications are digitized by agencies.
  • Centralized web application:The data of PwDs is available online across the country through a centralized web application.
  • Quick assessment:Hospitals and medical boards calculate the percentage of disability.
  • Online renewal and updates: PwDs or their representatives can renew and update information online.
